If you have already made your first appointment, you can preregister by clicking on the links below to download our New Patient Forms. Please print and complete the forms prior to your arrival. This will speed up your new patient registration.


We do not accept insurance. We can, however, provide you with a code sheet to turn in to your insurance to file for out-of-network benefits. Blood work can be run through insurance. Other functional medicine tests may require a copay in addition to your insurance.
